This measure helps evaluate fat distribution patterns, with higher ratios indicating relatively more visceral fat, which is associated with greater metabolic risk than subcutaneous fat.

Why this matters
A high ratio means proportionally more dangerous visceral fat, increasing metabolic and cardiovascular risk even at normal weight. People with high ratios ("apple shape") face greater health risks than those storing fat subcutaneously ("pear shape") despite similar total fat. High visceral proportion drives insulin resistance, inflammation, abnormal cholesterol, and hypertension without obvious symptoms. This pattern particularly affects men and postmenopausal women.
The ratio helps explain why some normal-weight individuals develop diabetes while some overweight people remain metabolically healthy.
