Frequently asked questions

A coronary calcium score, or CAC score, shows how much calcified plaque has built up in the arteries that supply your heart. You get it from a quick, low-dose CT scan, with no contrast agent. Calcium in these arteries is a sign of atherosclerosis, plaque build-up in the artery walls that can lead to a heart attack.

Your result is an Agatston score: 0 means no calcified plaque, 1–10 is minimal, 11–100 mild, 101–400 moderate and above 400 severe. Your report also puts your score in relation to your age, since the same score means more when you're younger.

It's most useful if you're roughly 40 to 70, with no heart symptoms or known heart disease, but with risk factors. These include high cholesterol, high blood pressure, smoking and early heart disease in the family. It comes into its own when your risk sits in the middle, neither clearly low nor clearly high. That's where the result can tip a decision, like whether to start a cholesterol-lowering medicine.

European and US prevention guidelines both name it as an option for exactly this situation. So it's mainly a test for this middle group, and for them a really valuable one.

It uses X-rays, but only at a very small dose: about 0.5–1 mSv. A standard chest CT gives roughly 5 to 10 times more. There's no contrast agent, so no injection either.

Like any X-ray scan, it carries a very small risk. That's why one of our doctors goes through your health history in a short pre-consultation to check the scan makes sense for you. If you're pregnant or might be, just let us know before booking.

Good news: no calcified plaque was found in your coronary arteries. For most people, that means a low risk of a heart attack over the next 5 to 10 years.

It isn't a guarantee, though. Early, soft plaque hasn't calcified yet, so it doesn't show on this scan. A zero is also less reassuring if you smoke, have diabetes or have early heart disease in the family. So keep looking after your cholesterol and blood pressure.

If you have chest pain, shortness of breath or palpitations, a zero doesn't rule out heart disease, so see a doctor now.

A high score means more calcified plaque and a higher heart attack risk, but not that one is on its way. There's a lot you can do, and your report explains your result and next steps.

A moderate or severe score, or one that's high for your age, is a good reason to work on cholesterol, blood pressure, blood sugar, smoking and exercise. Your GP or a cardiologist can help you decide whether a statin or another cholesterol-lowering medicine makes sense, and whether you need more heart tests.

With chest pain, shortness of breath or palpitations, see a doctor now.

No, that's not what it measures. It shows calcified plaque, which says a lot about your risk. But it can't show whether an artery is narrowed or how well blood reaches your heart. So it's a risk check, not a way to explain symptoms.

A CT with contrast agent (CT coronary angiography) shows narrowing, and a stress test shows blood flow. Your doctor arranges these when there's a medical reason.

If you have chest pain, shortness of breath or palpitations, don't wait for a scan: see a doctor now. For sudden or severe chest pain, call 144.

It takes about 10 minutes, and there's nothing to prepare: no fasting, no special steps. You lie on your back on the CT table, with small sticky pads on your chest to track your heartbeat. Then you hold your breath for a few seconds while the images are taken. It doesn't hurt.
